Outreach Worker, ITC Program, Ulverstone

The Outreach Worker,  Integrated Team Care Program, works under direction of the Care Coordinator Aboriginal Health to assist Aboriginal and Torres Strait Islander people to access appropriate health care services with a focus on chronic disease management.  Provision of outreach services which will include providing practical assistance to Aboriginal and Torres Strait Islander people to access health care services, distribution of information and resources and establishing and maintaining regular communication with the local Community. The role is based out of Ulverstone.
